Job Description:                 Social Mobilizer


  Meeting with communities regarding awareness raising of eye health Care
  Awareness raising session for community’s eye health care
  Mobilize community regarding eye screening BHUs, RHCs and THQ Levels.
  Meeting with SMO, MO, LHVs, optometrist and SH&N Supervisor Regarding Eye Screening
  Meeting with Social welfare department, CEO health, District Education officer and government officials regarding inclusion of persons with disabilities in Eye Health Care
  Need assessment and identification of person with disabilities
  To organized theater in education in Educational institutions concerning inclusive eye health care
  Provision of assistive devices to PWDs under the project of IEH

Mojaz Foundation Project Title “Diarrhea Reduction Initiative through Hygiene Promotion and Women Empowerment” Social Organizer (Districts: Narowal, Layyah and Sahiwal since January 2017 to April , 2019) Key Achievements • Develop linkage with line department like Education Health, PHED, Social welfare, and local government for smooth transition of the programme activity in the targeted area for prolong time. Had a chance to establish rural sanitary mart and worked on women empowerment component with leading donor partner • Provision of low cost latrine material in the community and ownership was shifted to the local community people in order to run this activity for the betterment of their village overall. • Close coordination and inclusion of EPI (Expanded programme immunization) in the current activity. • Poor & affected families were accessed and food was distributed. • IEC Material developed for EPI especially for Diarrhea, Hygienic practice • Successful training delivered (CRP, VWC, Teacher, WASH Club, Youth) Job Description (work as a team leader) ● Facilitate Field team for Formation of (VWC) ● To supervise field team to conduct and submitted report timely to DC ● To manage all filed activities as per agreed in implementation plan and provide support to field staff for proper implementation of field activities ● Developed linkage with line department like Education, Health, PHED, Social welfare, and local government. ● Delivered Health and Hygiene session On Seven Point Plan In targeted Community through BCC Approaches. ● Formation of district forum and organized meeting with ODF committee at district level. ● Support district coordinator to develop training agenda and manuals. ● Support district coordinator to prepare monthly and quarterly report. ● Support teams to develop monthly work plan. • Deliver training to Crossovers School Wash club, on WASH and 7 point plan

• I have been engaged with the government officials of District Muzaffargarh and Dera Ghazi Khan for the smooth implementation of “PUNJAB SAAF PANI” Project.
• I have been assisting the Project Coordinator in drafting Project Documentation including, Work Plans, Village profile forms, Detailed Implementation Plan and also arranging these documents in files.
• I have been visiting the community meanwhile for committee formation and also for the follow-ups.
