The Case Management Assistant at Shifa Foundation is responsible for engaging communities to promote nutrition interventions, improve health and hygiene practices, and ensure effective identification, referral, and follow-up of beneficiaries. The role focuses on community awareness, mobilization, and linkage between communities and programme facility centers (FCs) to support successful implementation of nutrition and health interventions..
Responsibilities
- To mobilize the community on nutrition intervention.
- To conduct health and hygiene sessions to sensitize the community on healthy practices to implement them in their daily life.
- To conduct Union council and village mapping
- To identify target community and mobilize community groups
- To establish new contacts with communities
- To do follow-up of SAM/MAM and overdue cases
- To refer new benefices to FCs
- Any other task assigned by the Line Manager
