Job Title: Lady Health Visitor

Objective: To implement the HDF Health Program in the Region as per HDF Health Policy.
Reporting to: Health Coordinator

Responsibilities:
1. To assist HC in patient’s examination in OPD at CHC.
2. To assist HC in ANC, PNC, FP, PAC and nutrition counselling and contraceptive method dispensing /insertion/referrals .
3. LHV will assist HC in labor & childbirth.
4. She is responsible to process instruments according to instrument processing guidelines.
5. Under supervision of HC, She will facilitate referral of high risk/critical patients to nearby appropriate health facility, according to referral guidelines.
6. Ensure & supervise the daily visits of community health workers.
7. LHV will have to visit the community thrice a week ,During her visit she will perform the following tasks ,
I. Examination of pregnant and post-natal women to identify high risk patients.
II. Neonatal examination to ensure immediate care of newborn.
III. Ensure TT immunization of WRAs and pregnant ladies.
IV. Ensure immunization of under 5 under EPI schedule.
V. To asses/inquire women’s knowledge information about preventive healthcare, Exclusive breast feeding, IYCF, Weaning and facilitate activities to enhance their knowledge.
VI. Record & report data.
8. LHV can visit HHs of CHWs for inquiring about the performance of CHW & has to fill the Monitoring form from 6 house hold, during her supervisory/monitoring visit she will perform antenatal, postnatal checkups, Newborn examination in same neighborhood.
9. LHV will assist HC in the implementation of
I. School Health Program.
II. To conduct Medical Camps.
III. To conduct Health Seminar.
IV. To conduct health session with CHWs.
She will ensure
10. Immunization under EPI schedule.
11. Record keeping of children (under 2 years of age) TT vaccination & WRAs.
12. Growth Monitoring of children (under 5 years of age).
13. In case of maternal ,neonatal and infant death ,She will fill VA tool/questionnaire at deceased house under the supervision of HC and will report and keep the record.
14. LHV will assist HC in making the medicines requisition and expense of medicines.
15. LHV will assist HC in the maintenance of Health Fund.
16. She will assist HC to prepare, Analysis ,correction & submission of MPR regional management
17. Maintenance of field visit reports on specific formats in (English OR in Urdu).
18. In absence of HC, the most Senior LHV will be the acting in charge of CHC for administrative duties.
19. Carrying out all the assigned annual operation plan based activities in program areas in supervision of HC & Regional Management
20. Any other duty assigned by organization.
Qualification: Two years LHV course from Pakistan Nursing Council recognized institution. Registered with Pakistan Nursing Council and two years of working experience.
