Denizard is seeking a dedicated and compassionate Remote Care Coordinator to join our Nephrology Chronic Care Management (CCM) Program. This role focuses on providing comprehensive care coordination for patients with chronic kidney conditions by facilitating effective communication between patients, healthcare providers, and interdisciplinary teams. The ideal candidate possesses excellent patient education skills and is experienced in using electronic health records (EHR), telehealth platforms, and remote patient monitoring tools to enhance patient care outcomes. With a minimum of one year of experience in healthcare coordination or chronic care management, the candidate will support patient engagement and adherence to care plans while ensuring seamless documentation and compliance with clinical protocols. The Remote Care Coordinator works independently without managing a team but collaborates closely with nephrologists, nurses, social workers, and other healthcare professionals. Strong communication abilities, empathy, and kindness are essential qualities to foster trust and motivate patients in managing their chronic conditions. This role requires proficiency in telehealth communication and chronic care management software to deliver timely interventions and monitor patient progress remotely. Responsibilities

Coordinate and manage care plans for patients enrolled in the Nephrology CCM Program to ensure continuity of care and adherence to prescribed treatments.
Utilize electronic health records (EHR) and chronic care management software to document patient information accurately and maintain up-to-date clinical records.
Conduct patient education sessions remotely to inform patients about their condition, treatment options, medication adherence, and lifestyle modifications.
Facilitate telehealth communication between patients and healthcare providers, ensuring timely scheduling of virtual visits and follow-ups.
Monitor patient status through remote patient monitoring (RPM) platforms to identify changes in health conditions and escalate concerns appropriately.
Collaborate with interdisciplinary healthcare teams, including physicians, nurses, social workers, and dietitians, to develop and adjust individualized care strategies.
Provide empathetic support and counseling to patients to improve engagement and self-management of chronic kidney disease.
Ensure compliance with clinical guidelines and program protocols while maintaining patient confidentiality and data security.
Identify barriers to care and work with patients to develop solutions that improve access to services and health outcomes.
Prepare and submit regular reports on patient progress, care coordination activities, and program metrics to Denizard management.
