District Field Supervisor

Job Description

The incumbent will be responsible for the overall implementation of project activities in the assigned district in accordance with the New Funding Model (NFM) Operational Guidelines. The incumbent will work in close coordination with the Regional Coordinator and Project Assistant.
·       To coordinate with District TB Coordinator, Regional Coordinator, District Lab Supervisor, Private Healthcare Facilities and Private Labs for effective implementation of the TB-DOTS under PPM project.
·       Responsible for carrying out district mapping exercise in consultation with the district health department (Executive District Officer-Health (EDO-H)/District Health Officer (DHO)/ and District TB Coordinator (DTC) under guidance of the Regional Coordinator
·       Timely submission of monthly work plan, monthly progress report, and quarterly TB Recording and reporting (R&R) tools after endorsement from district health authorities
·       To conduct regular visits of selected private health facilities and labs to ensure proper recording of TB R&R tools , timely supply of anti TB drugs, labs reagents, chemical and registration of TB cases
·       To do contact screening of the bacteriologically positive case on regular basis
·       To support health facility staff in maintaining anti-TB drug stock
·       To develop regular follow up mechanism for the identification of TB suspects, MDR suspects and registration of new smear positive cases
·       Responsible for the implementation of the PPM activities in the field in close coordination of district health authorities and in support of the Regional Coordinator 
·       Responsible for maintaining chronological record (programmatic and financial) including activity photographs, minutes of meetings and field monitoring reports etc. of all the field activities at district level
·       Responsible for submission of programmatic and financial record to regional office of all field activities on monthly basis in line with the NFM Operational Guidelines
·       Conduct him/herself professionally and personally in such a manner so as to bring credit to (name of SR) and in no way jeopardizing its humanitarian efforts·
       To support and facilitate supervisory visits from the national, provincial and district level.
·       Any other task or responsibility as assigned by the Regional Coordinator

 8000 chilled enrollment 
Control the dropout through community mobilization 
Formation of PTSMC and arranged the trainings
 Formation of Students Groups School levels 
Conduct the Teacher Training
Prepare case studies in the project area
Prepare the weekly and monthly planning. 
Prepare daily weekly and monthly reports 
Follow ups field visits SDPs
Any other task assigned by the supervisor.

Project provision of WASH facilities to 3000 families of flood 2015 affected areas. Funded by concern USAID
 Key Responsibilities

Identification affected flood 2015 families
 Formation of HQMC and WASH committees and signed the TOP
 Formation of O M and arranged the trainings
 Assessment 300 hand pump and latrine
Conduct the health hygiene sessions Prepare case studies in the project area
Prepare the weekly and monthly planning. Prepare daily weekly and monthly reports
 Follow ups field visits and hand Over of hand pumps and latrine
Any other task assigned by the supervisor.

Project Rehabilitation Support to Flood Survivors In Jaffarabad Project in 4 UCs Funded by TDH
Key Responsibilities

Carryout Social need assessment.
To arrange new area opening meeting.
Prepare the Social mapping of village village Profiles.
Establish VCs, ACCs and TCCs. Conduct the base line survey.
Conduct the well being exercise. Establish ACCs an umbrella organization of village committees .
Establish disaster management committees at VC and ACC level.
Conduct accountability forum meeting at UC ,
Tehsil and Dist Level .
Facilitate training on SOPT. CMST record keeping and LMST.
Conduct Case Studies regarding Social mobilization.
Carry out field activities as per work plans. Carry out need assessment of flood 2010 affected communities.
Identification of families in the field area.
Awareness raising as par project guidelines.
Ensure provision of timely support to the project field area communities.
Prepare case studies in the project area Follow ups of relief interventions.
Prepare dailyweekly and monthly reports.
Any other task assigned by the supervisor.

- Project Early Recovery of Flood Affected Communities Project in 3 UCs Funded by TDH
Key Responsibilities

Carry out Social need assessment. To arrange new area opening meeting.
Social mapping of village village Profiles.
Conduct the base line survey.
Conduct the well being exercise.
Establish VCs, and ACCs
Identification of families in the field area.
Conduct the GALs trainings with females
 Identification of 300 poor families for goat and poultry distribution
Prepare case studies in the project area
 Follow ups of relief interventions.
Prepare dailyweekly and monthly reports.
Any other task assigned by the supervisor.

Project ERF WASH Project Flood 2010 Affected Area Funded by ERF
Key Responsibilities

 Identification and assessment of affected flood 2010 2400 families
Formation of HQMC and WASH committees
 Finalizes of 2400 families list and tokens
 Distribution of 2400 hygiene kit and Pit Latrine
 Identification 100 location for hand pumps.
Conduct the Health Hygiene Sessions
 Prepare case studies in the project area
 Follow ups of relief interventions.
Prepare dailyweekly and monthly reports.
Any other task assigned by the supervisor.

Project Livelihood Project Flood 2007 Affected Areas Funded by WFP 
Key Responsibilities

Identification and assessment flood 2007 affected 2000 families for food package distribution .
Token and food package distribution.
Distribution cash for work flood affected families. Identification distribution of shelter materiel
. Identification for rehabilitation water pound and water course in flood affected areas Follow ups of relief interventions.
Prepare dailyweekly and monthly reports.
Any other task assigned by the superviser
