Purpose of the role
Responsible for CMAM; community outreach component activities, establishment community nutrition sites ensure proper screening, MM supplementation; create effective referral system, implementation of SBCC on health nutrition awareness and community support group capacity building under the supervision of Nutrition Officer
Individual accountabilities
Responsibility to ensure the effective implementation of CMAM community outreach activities in the field.
Develop micro plan of community outreach activities and submit on monthly basis.
Community sensitization on the program objectives and methods to stimulate & eventually promote the understanding and participation of the target population. Community sensitization starts at village- and Community-leader levels and eventually extends to the entire population.
Establishment of community outreach setup at each union council through selection of community volunteers and health workers.
Community mobilization and beneficiary screening, provision of MM Supplementation and follow up.
Establishment of strong referral mechanism between community to health facility OTP/CMAMI site and follow up.
Establishment of nutrition community support groups (MTMSG,FTFSG, CVG) at each union council in consultation with community.
Support to the Community Health Worker (CHW) and Volunteers during screening (active case finding) and linking up the MTMSG , FTFSGs with the LHW program
Provide technical support to CHW on mobilization, active case finding, defaulter tracing and home follow up visits through follow up trainings and on job coaching and supportive supervision
Ensure community social behavior change communication health and awareness session/campaign on regular basis through CHW/support groups.
Establish linked and meetings with the Community leaders and different group existing in the working area.
Job Description:
Responsible to share Morning Assembly Report and evening reports.
Responsible to conduct UC wise Meeting and training for Team members.
Close coordination with Health department and DPCR and Line Manager.
Participate in DEOC meeting, prepare and present presentation of previous Polio campaign and finalize minutes of meeting.
Carry out field monitoring visit to ensure that the Vaccination are properly Done in field and data entry is being done on telly sheet.
Provide on-site technical support and capacity building to field teams for implementation of project activities.
Prepare field monitoring reports on daily basis.
Conduct desk Validation and field validation of Microplan and other requried documents.
Responsible to submit report team visited but missed the child (TVBMC), Not at home (NT House) Wall chalking and Finger Marks.
Follow-up on a regular basis to ensure timely achievement of targets in line and Find zero dose children, covered children and un vaccinated children.
Review, verify the Microplan prepare Uc summary list.
Responsible for compiling and analyzing data and developing summary reports and share with Polio Eradication officer.
Support to Polio Team and Area Incharge and other review missions.
