Introduction:

Social Capital Development, Education & Literacy, Primary Healthcare, Livelihood and food Security, and Sustainable Environment are key pillars of our program.
Objective: This position is responsible for overseeing all healthcare activity and implementing the HDF Health Program in the Region as per HDF Health Policy.
Key Responsibilities:
- Health Coordinator (HC) will ensure implementation of all activities /Tasks of the health program at the Community Health Center (CHC) and field are aligned with HDF policies, SOPs, and Standards of quality.
- Support RPM in preparing DIP of AOP.
- Ensure smooth implementation of AOP (Health).
- Examination of outdoor patients at CHC and Mobile Health Units/ Medical Camps.
- Responsible for bookkeeping/record and data management.
- Referral of high-risk patients to nearby appropriate health facilities.
- HC will ensure the
- Daily visits of CHWs.
- Scheduled visits of LHV.
- Monitoring visits to the program area to assess the performance of LHV and CHWs in the field.
- HC will visit the field ONCE a week.
- Implementation of School Health Program.
- To facilitate LHV in record /data keeping as per standards.
