Identify /Manage/ referral high risk cases
Maintain good coordination among team & with Government staff
Ensure Health education sessions especially.
Record all health activities & report in line with set indicators
Conduct monthly coordination meetings at the facility with internal and external team members
Analyze monthly/ Quarterly data & improve clientage ratio at her facility to achieve set targets
Ensure submission of reports daily/weekly/monthly as per reporting requirements
Ensure & execute Infection prevention & Hospital waste management protocols at facility adopted at all levels
