<div><p id="docs-internal-guid-4ce5789a-7fff-9c5d-c527-63b8ec59d23e" dir="ltr">CHIP TRANING &amp; CONSULTING, PTPP PROJECT WHO</p> <p dir="ltr">                                                                      (01-08-2015 to 28 Feb 2021 )</p> <p dir="ltr">UNION COUNCIL POLIO OFFICER</p> <p dir="ltr">I’M WORKING IN THE FOLLOWING SECTIONS:</p> <ol> <li dir="ltr"> <p dir="ltr">SIA’S Polio Campaign Routine Immunization, AFP Surveillance, Social MOBILIZATION ACTIVITIES. </p> </li> <li dir="ltr"> <p dir="ltr">Monitoring &amp; Supervision of Pre-Campaign Activities</p> </li> <li dir="ltr"> <p dir="ltr">Monitoring &amp; Supervision of Intra-Campaign Activities</p> </li> <li dir="ltr"> <p dir="ltr">Monitoring &amp; Supervision of Post-Campaign Activities</p> </li> <li dir="ltr"> <p dir="ltr">Monitoring &amp; Supervision Routine Immunization</p> </li> <li dir="ltr"> <p dir="ltr">Social Mobilization Activities AFP Surveillance </p> </li> </ol> <p dir="ltr">Other Job Responsibilities are: -</p> <ul> <li dir="ltr"> <p dir="ltr">Monitoring &amp; Supportive Supervision Pre- Campaign Phase Supervisors, UCMOs &amp; AIC’s Training Ensure 100% Attendance, Facilitation UPEC Meeting UC Level, Discus Last Campaign Issues &amp; Observation with Chairman UPEC &amp; Planning UP Coming Campaign, Ensure Micro planning Validation &amp; Support UCMO &amp; AICs for Preparation Updated Micro plan all components, Social Mobilization Pre-Campaign Mosque Announcement Religious Persons, Influencer Meeting Regarding Supporting in Polio &amp; Routine Immunization.    </p> </li> </ul> <ul> <li dir="ltr"> <p dir="ltr">Intra-Campaign Monitoring, Early Morning arrived at Team Support Center Monitor Teams Deployment Morning Orientation, Immediate report to DPEOs &amp; DPCR if found team absent replace, Monitor Fix team filling checklist, in field Monitor Mobile Teams, T.P Team Supervisors and take weak area H-H Clusters for checking the quality of work, any observation issues found in the field on job training given by team &amp; Immediate Report to DPEOs, DPCR &amp; Concerned. Daily Basis attends Morning &amp; Evening Meeting at UC Level and Evening Meeting at Taluka level share field-level issues &amp; observation and Support Union Council Medical Officer for Compilation Daily Basis report, Support &amp; Trained Area In charges for any issues.     </p> </li> </ul> <ul> <li dir="ltr"> <p dir="ltr">Monitor &amp; Supervision and Collect Post Campaign Data analysis validation Missed children cover during campaign support, Market Survey (LQAS) and Support External &amp; Internal Monitors during Field Activities if found missed children validation in field and report &amp; reason for missed children immediate to DPEOs &amp; DPCR and other Concerned for further necessary action.     </p> </li> </ul> <ul> <li dir="ltr"> <p dir="ltr">Monitoring &amp; Supervision Routine Immunization on Daily and Weekly Basic Support UC assign Vaccinator for Routine Immunization, Social Mobilization refusals covering in UC meeting with Religious Person influencers Political Leader for Supporting in Routine Immunization Vaccination Supporting in Micro planning Monthly reports graph chart updating  attend weekly &amp; Monthly Meeting Routine Immunization at DPCR Chair Meeting honorable Deputy Commissioner, Participated by DPEOs, NSTOP Officer, DHCSO, DHO sahib, DFP EPI, DSV EPI &amp; TSV EPI, Share Field Level all issues with the chairs on PowerPoint presentation   </p> </li> </ul> <ul> <li dir="ltr"> <p dir="ltr">AFP Surveillance &amp; Social Mobilization Activities, Conduct AFP Surveillance Session in Every Village on Assign Union Council, Weekly meeting with Heath Facility Staff ICMO, informal &amp; non-informal brief AFP Case &amp; Provide case definition, and Support DSO, DPEOs for investigation AFP Case, and immediately report if found AFP Case, Meeting With religious persons, villagers, influencers, health-related persons for AFP Case if any. </p> </li> </ul> <p dir="ltr">ADDITIONAL TASKS PERFORMED INDIVIDUALLY</p> <p dir="ltr">1.    Supporting DPCR if any Assignment is given by DPEOs DPCR for Data related, Micro planning Desk Validation &amp; Field Validation &amp; DPCR and Field Level,  </p> <p dir="ltr">2.   Meeting Every Week and Month with DPEOs at WHO office Submit Weekly and Monthly Report to DPEOs and given Monthly Presentation at WHO office and share performance.</p> <p dir="ltr">3.   Support Taluka Focal Person in Evening Meeting for Collecting Results of Taluka Kashmore   </p> <p dir="ltr">Zone B, Compiling Taluka Level UCPOs &amp; TTSPs issues &amp; UCMOs issues at Taluka Level and Daily Basis email to DPEOs &amp; DPCR for solving issues.</p> <p dir="ltr">4.   Attend Intra Campaign Daily Evening Meeting with UCPOs &amp; TTSPs Collect Reports and validate all daily basis formats filling in the field and submit all report in WHO office.</p> </div>
