Social Support Coordinator
• Coordinating & enforcing smooth & proper functioning of all social support activities at the Programmatic Managements of drug Resistant TB (PMDT Sites ) INDUS HOSPITAL / JPMC / QUETTA / SUKKUR / ICD HYDERABAD KOTRI / MIRPURKHAS/Nawab Shah/Larkana/Loralai, Turbat
• Strictly following infection control guidelines DOST & DR-TB patients
• Baseline intake meeting with all DR-TB patients at the time of enrollments
• Responsible for ensuring timely Provision of food basket Voucher’s issue to DT-TB Patients and Treatment Supporter .& Patients inventive disbursement on monthly follow ups MDR patients,
Nutritional Support for DRTB patients having low BMI suggested after evaluation by Nutrionist.
• Ensure Food basket voucher’s issue is provided to patients and Treatment supporter who are Compliant
• Liaise with clinical, filed team and lab to ensure patients and treatment Supporter compliance.
• Maintain and complete documentation summary of FBV and Travel incentive provided to patients and treatment Supporter’s will be sent to NTP with social support Vouchers.
• Maintenance of Record in an auditable manner.
• Make sure that all patients received FBV and Traveling incentive within the validity period of vouchers
• Liaise with nutritionist to ensure patients with Low BMI receiving nutritional supplement.
• Monitoring field visits and phone calls to patients to ensure no issue with food baskets voucher
• Weekly presentation on updated of Social Support
• Sent Social support data NTP monthly and Quarterly basics
• Ensuring the Social Support Data is maintained at ENRS & DR-TB patient.
• Coordinate All PDMT Sites TB MDR Mounting / Evaluation /Treatment Supporter Mounting & Food Basket Voucher Issues and Travel Incentive to DR patients and Treatment Supporter at PMDT Sites: Quetta, Larkhana, Banzeerabad Sukkur, Mirpur khas ICD Kotri IHK Karachi
Liaise with clinical, field team and lab to ensure patient and TS compliance.
