• TSM will be responsible to carry out contact tracing of Drug sensitive TB (DST) and Multi Drug Resistant (MDR) Patients of concerned Tehsil /Taluka
• TSM will work closely with concerned LHWs and Community Health workers to identify and locate contact DST and MDR patients
• TSM will devise field visit plan to visit assigned area for contact screening in coordination with LHWs and CHWs under guidance of District Field Supervisor and District Coordinator (DC)
• TSM will assess and understand the assigned work area to trace suspected DST and TB MDR patients, collect sputum and hand over to nearby Health Facility where Gen Xpert machine is available for timely diagnosis
• TSM will ensure registration of positive contact with nearest Treatment BMU and PMDT Site
• TSM will develop liaison / coordination with nearby health facility staff /team where TB case management is available or Gen Xpert machine is available
• TSM will facilitate and maintain follow up of diagnosed DST /TB MDR patient of his/her area for adherence to management and regularly report to his/her supervisor
• TSMs will provide a brief update on his/her field activities regularly on daily bases to his/her Supervisor (DFS).
• TSMs have a central role in contributing their efforts to achieve set targets of project activity.
• TSM will also facilitate the training activities relevant to his/her job roles /responsibilities offered by the organization / program.
• TSMs also report interesting stories related to DST and TB MDR patients if he/she noticed in his/her assigned work area for record and learning experience
• TSMs will remain engaged to their work plans and continue working with their supervisor, and will also share experience / inputs to strategic planning of detecting DST and TB MDR patients.
• TSM will report to DFS and District Coordinator
Ensure the smooth implementation of field activities under the COVID-19 emergency response project to support off-camp Afghan Refugees in Peshawar, Nowshera and Swabi, Khyber Pakhtunkhwa, Pakistan.
Social Mobilizer will be responsible for mobilizing the community through dialogue, awareness raising, distribution and using PRA/RRA tools to identify causes of local problems and their solutions.
SM will support in beneficiary selection, preparation of lists, vouchers distribution and follow-ups in the target communities and districts.
SM will identify and proactively communicate any issues that might affect the project implementation and engage community support with guidance from the Project Coordinator and Project Mobilization Officer to address the issues.
SM will ensure active community support for the project activities and ownership.
SM will be responsible for collection and dissemination of information between communities, RI project staff and stakeholders.
SM will provide facilitation in the community groups’ formation, record keeping, weekly meetings and training/ capacity building of the community group members, if needed.
Assist PC to ensure completion of the project in a timely manner and the budgetary limits.
Prepare field reports on a timely basis.
Maintain and manage record of all field related documents in hard and soft forms.
Support PC and project team in the mapping, identification and selection of the beneficiaries in target districts as per agreed mechanism.
Work closely with the PC, PMO, M&E and project team members to ensure that all activities are executed in a coordinated and timely manner.
Represent RI at community level coordination meetings and ensure close coordination and collaboration with stakeholders.
