· Ensure Coordination with staff in Hospitals

· Assist in preparation of District Map, AFP Case Map, WPV, VDPV cases, active surveillance and zero reporting Map, AS visit planning-weekly, HRMP Map with indicators, surveillance indicators HR UCs with indicators.
· Manage AFP case files district line list cross notifies AFP line list, EX AFP line list, Epid studies, AFP cluster investigation report.
· Participate in district surveillance review meeting and draft minutes.
· Prepare visit Plan.
· Conduct presentations and DTF presentation.
· Orientation regarding surveillance and AFP case
· Establishment of an end-to-end specimens tracking system from the health facilities to the National/ International laboratories.
· Ensures that the measures of Biosafety in the laboratory of the INSP are well implemented.
· Establishment and maintenance of a complete contact list of provincial and health facilities surveillance officers.
· Follows up for the corrections and information updates identified with field investigators and notifying facilities, and documents them accordingly.
· Update of AFP, Measles, Neonatal Tetanus and Yellow Fever cases investigated in DHIS2 platform.
· Involved in District level Planning for Polio Vaccination Campaigns
· Attending District Polio Eradication Committee (DPEC) meeting under chairmanship of Deputy Commissioner
· Monitoring of Polio vaccination teams in the field.
· Assist in Preparation of Micro Plan for Polio vaccination Campaigns
· Micro plans Quality Assessment (MPQA) of Union Councils in the district.
· Intra campaign monitoring at district level
· Taking samples (cluster) to identify the missing and refusal children.
· Support teams for conversion of the Refusal families against polio vaccine
· Preparation and monitoring of reediness meeting for Polio vaccination campaigns
· Attending evening meetings and give positive feedback of field observations for the betterment of the program
· Monitoring teams on catchup day for missing children.
Post campaign monitoring
· To supervise the team in overall activities and follow up the team work.
· Carry-out Observational Survey & Social Mapping of the target areas.
· Conduct the Baseline Survey of the target areas with supervisor.
· Organize & conduct trainings, meeting & dialogues of the target group.
· Initiate & regularize activities on Non-formal education, awareness raising & Health & hygiene session of the vulnerable groups.
· To facilitate formation of community groups & initiate their capacity building process.
· Provide support/facilitate the social organizer to ensure the conduction of Situational Analysis at targeted areas/villages by using PRA, RRA tools.
· Organizing Community Based Organizations (COs) and mobilizing the communities for carrying out the project activities.
· Conduct Participatory Resource Constraints Analysis (PRCA) at COs level according to all project components through ensuring development of Population Chart, Import & Export Chart, Poverty Score Card analysis and need identification and prioritization in the communities.
· Participating in the preparation of benchmarks and monitoring plan for all the project components at CO’s level. Facilitate the organized communities in formulation of Micro Investment Plans (MIPs) at household level.
· Facilitate and provide support to the social organizer and technical field team i.e. field engineer as well as communities to formulate the Operation and Maintenance committees (O&M), Project Planning, Implementation and Monitoring and Evaluation.
· Facilitate and provide support to the social organizer and regional team in the Training Need Assessment of the potential trainees at COs level and ensuring the selection process of the trainees for capacity building and Livelihood trainings prescribed under RAHA programme.
· Conduct and participate in the follow ups of COs against the benchmarks and according to the monitoring plan for all the project components.
· Regular develop and provide monthly, weekly and daily work plans by ensuring regional approvals as far as submit monthly progress reports against the targets.
