Provide care and support to PLHIVs (People living with HIV) and their families in collaboration with other team members and the management.
Create and strengthen referral mechanisms for ARV/CD4 and all health related services for PLHIVs.
To create enabling environment and reduce HIV related stigma and discrimination toward PWID and their families, within the existing ART, PPTCT and public sector hospitals.
Work in close collaboration with the designated team members to develop integrated continuum of care.
Establish and maintain contact with concerned governmental and non-governmental organizations.
Coordinate with the CoPc+ staff to ensure timely referral of HIV positive clients to the relevant services as and when required.
Undertake regular performance review to ensure smooth implementation of social mobilization services and quality of care.
Assure that service provision complies with national guidelines and treatment protocols.
Maintain a regular follow-up with the clients to ensure adherence to the AIDS treatment.
Conduct review meetings, and promptly resolve problems as they occur in collaboration with COPC+ management team.
Ensure quality of client monitoring data, utilization of available data, and timely submission of reports.
Ability to understand the positive impact of services on Target Population (spouses and sexual partners of PWIDs)
Effectively implement the assigned project related activities as per the approved work plan and against set targets
Register spouses of IDUs as per policy and keep a copy of their registration record and also coordinate with DEO on daily basis.
Conduct daily counseling sessions with spouses of IDUs on issues including HIV, safer sexual practices, health & hygiene, role of family in clients care, and also motivate them to get tested for HIV.
Distribute syringes( wherever needed) and condoms among spouses of IDUs as per project policy and maintain daily record and hand it over to the designated staff member
Refer spouses for medical services to the medical team
Refer spouses for HTC to COPC site.
Refer spouses to PPTCT programs whenever required.
Develop reports of daily activities
Ability to follow the instructions, guidelines, processes and procedures for Prevention and Care of spouses of IDUs in compliance with defined objectives.
Follow the referral guidelines and ethical protocols while working in the field.
Coordinate with other staff members to have insight into the issues related to performance and regularly report to the Site Manager for guidance and technical support
Close coordination and linkage with public sector health facilities and CHBC sites for provision of defined services
Regularly update data collection and report to the Site Manager.
Identify and highlight sensitive and important areas of concern regarding spouses of IDUs so that those areas can be further worker out for research or service delivery.
Perform any other tasks identified by the Site Manager that may be necessary to ensure that the Program is implemented effectively.
Survey and sensitize the communities about the microfinance packages and offers
Organize the registered members into small groups
Regulate the activities of the group members
Issue out loans and ensure the pay back of loan installments
Carry out regular field visit, conduct group meetings, and assess potential borrowers
Prepare weekly and monthly report on the activities and submit it to the Branch Manager
